Waiver/Acknowledgement

I acknowledge and accept that Martial Arts and boxing are dangerous sports and involves the risk of serious injury. I hereby warrant that I am physically and medically sound to exercise and have voluntarily elected to do so.

I acknowledge that whilst at the Academy, both my property and my person shall be at my own risk and I will not hold Adapt Academy, its staff or volunteers liable for any personal injury or loss of property arising whilst on these premises.

Adapt Martial Arts & Boxing Academy teachers/instructors are at a minimum of a Black Belt level. Assisting teacher/instructors’ are volunteers working toward their Black Belt.

Media Consent

I hereby give permission for Adapt Martial Arts & Boxing Academy to capture and use photographs, video, or audio recordings of myself and/or my child. This may include full or partial recordings, with or without accompanying text or illustrations, for use in Adapt Academy’s promotional materials, publications, presentations, and online platforms.

This consent extends to electronic and internet-based marketing, including social media, and is granted for the purpose of positively promoting Adapt Martial Arts & Boxing Academy and its programs.

In any instances where names may be included, only first names will be used.

If you do not wish for yourself or your child to appear on social media, please let us know and we will do our best to ensure you are not featured. While we make every effort to respect these requests, we appreciate your understanding that occasional human error can occur.
